1. Identify the specific URLs or material that has been removed: Provide a list of the URLs or material that has been removed or to which access has been disabled and the location at which the material appeared before it was removed or access was disabled.

2. Provide your contact information: Include your name, address, telephone number, and email address.

3. A statement under penalty of perjury: Include a statement, made under penalty of perjury, that you have a good faith belief that the material was removed or disabled as a result of mistake or misidentification of the material.

4. Your electronic signature: Sign the counter-notification electronically or physically.
